PSYCHOLOGICAL TEST REPORT

II. GENERAL OBSERVATIONS
The client was about 5’ 5’’ in height with fair skin and medium muscle tone. His head was shaved and appeared to be pale. He has a tattoo on his left hand and left arm representing his gang and his clan respectively. The client has a pleasant personality, was cooperative and cheerful.
The client was informed of having mental problems and was bought to NCMH escorted by his jail guard on May 1, 2008 from Leyte City Jail. He stabbed and killed his two-year old son and was charged with Parricide. The client was already imprisoned twice in Leyte for two counts of physical injury. He was also rehabilitated twice for his drug use and twice for his mental illness between 2005 and 2006. The client believes that he is normal because he is not anymore thinking about unwanted acts and is now able to control himself.
The client’s illness started in 2006 while in the bus on his way back to Leyte. He started having dreams of a being (Kapre) telling him to kill his son for the protection of the human race. This being controlled the client’s mind and body leading him to stab his son twice on the chest. After which, he slashed his own wrists and stabbed himself twice on his chest. The incident happened on September 2006. He was imprisoned on October 1, 2006 until his recent transfer to NCMH. The client was advised to stay at NCMH for three months before attending the hearing.
The client was a consistent shabu drug user for twenty years; starting when he was still in first year high school. Unlike before, he can now sleep and eat well.
The client is 33 years old with two sons (one deceased) from a past relationship. He worked at 711 Alabang as a security guard. His parents had been separated for 25 years now and he is the second child in a family of five.
The client seems to be aware of his environment, projects a normal affect and maintains good eye contact.

Full Scale IQ: 71
Classification: Borderline
Pre-morbid IQ: 77 – Borderline
Sub-test Analysis:
The analyses of the client’s numerical reasoning and deductive reasoning capacity were fair. While analyses of the client’s concentration span, visual motor speed, long term memory, social judgment, logical reasoning, social intelligence, inductive reasoning capacity and learning ability were poor. On the other hand, analysis of the client’s visual memory was impaired.
